PAM account management failed: Authentication token is no longer valid; new one required

It was working well for me until yesterday when I logon to my OpenVPN, with Google Authenticator, I encounter this error:

API METHOD: authenticate
AUTH_RETURN
status : FAIL
reason : PAM account management failed: Authentication token is no longer valid; new one required
user : myusername

How do I get a new token?

I tried reset my password, tried reset my Google Authenticator to no avail.

What PAM modules are you using?

Also, are you enabling Google Authenticator support via the OpenVPN AS options or are you using a PAM specific module to enable it?

The error reads like a problem took place during the PAM stack rather than a OpenVPN AS specific error.
