Does Sent/Receive reflect real usage?

The server shows the client usage as sent and received.
On the server side, the Internet connection is not asymmetric, it's a full 1000G connection to the next.
However, on the client side, it could be cable based asymmetric or it might not be depending on the local connection.

Therefore, is the amount of data being calculated at the server end a true measure of data usage for all clients?
And if it is, how should I calculate the total usage? Do I add up both sent/received or just one?
